Register with us as a New Patient

Registration with the practice is preferably done online using the process below. If you cannot complete the online registration process please come into the surgery to fill out hard copies of the form.

Registration process:

  1. Check if you live in the area covered by our practice by looking at the map below.
  2. Complete a National Registration Form (link below) and save to your files
  3. Complete the Questionnaire Form (link below) and also save to your files.
  4. Attach BOTH the completed forms, a copy of photograph ID AND proof of address to an e-mail and send to: [email protected]

A separate form is required for each individual requesting registration.

Your previous medical records may take several weeks to arrive at the Practice.

Refusal to join the Practice

On occasions the practice does have the right to refuse a patient wishing to join the list. Such occasions are:

  • A patient having previously been removed from the practice list due to an irreconcilable breakdown in the doctor/patient relationship.
  • A patient with a history of violence.
  • A patient who is from a country where there are no reciprocal arrangements and therefore not entitled to free NHS care.

If you wish to change your GP practice or if you have been removed from a GP’s list, and cannot get another doctor to accept you you can telephone NHS Lothian registration department on 0131 275 7171 for advice.

People who are homeless may access local GP services. Alternatively GP, nursing and mental health services are available at TAP (The Access Point), 17 Leith Street, Edinburgh and at the Cowgate Clinic, 20 Cowgate, Edinburgh. Advice regarding housing and social work services are also available at TAP (The Access Point).
