Registering with the Practice
Enquiries to register can be made by telephone or visiting the practice – the receptionist will be able to tell you if you reside in the practice area. On registering with the practice you will be given a new patient questionnaire to complete and be asked to make an appointment with the health care assistant who will carry out a new patient check. You may also be asked for proof of entitlement to free NHS services and also proof of address. Should you have any chronic diseases e.g. diabetes, asthma, ischaemic heart disease, you will be directed, at this appointment, to the appropriate services in order that you will receive the best possible care.

Refusal to join the Practice
On occasions the practice does have the right to refuse a patient wishing to join the list. Such occasions are:
- A patient having previously been removed from the practice list due to an irreconcilable breakdown in the doctor/patient relationship.
- A patient with a history of violence.
- A patient who is from a country where there are no reciprocal arrangements and therefore not entitled to free NHS care.
